Please, if you cannot access our Internet based 'Patient Portal' Computerized Medical Record System, to input your essential medical information into your 'computerised medical record chart' ... you need to call our office and we will FAX or if necessary MAIL a paper copy of your paper Patient Forms - that need to be filled out completely and should be returned to us BEFORE your office Consultation & BROUGHT WITH YOU to the office visit - to ensure the BEST EXPERIENCE during your office Consultation time.
